- New members need to apply within 30 days of joining the Chamber. Sole proprietors must be members for 60 days before becoming eligible for Community Blue and Univera.
- New employees need to apply within 30 days of hire or within 30 days of becoming eligible to receive insurance through their company as per company guidelines.
- Employees need to work a minimum of 20 hours per week
- Original applications must be received by our office as follows: Independent Health, Univera, and The Dental Shop - at least 30 days prior to the first of the month effective date. Community Blue - at least 45 days prior to the first of the month effective date.
- All plans are billed quarterly and subject to a processing fee.
